Civil Practice Formulaw Suite

This Formulaw suite includes the essential forms for opening a civil case in Illinois.
  • Formats:
    • Formulaw

PLEASE NOTE: You will need to own or purchase TheFormTool PRO software tool, available at https://shop.theformtool.com//, to use the automated features of this form. This software works better on a PC than a Mac.

This suite of 21 essential forms for your civil practice comes from IICLE®’s trusted library of publications. The Civil Complaint covers a range of scenarios: in addition to including different Prayers for Relief for personal and nonpersonal injury lawsuits, this form allows for the following Plaintiffs and Defendants: individuals, individuals doing business under another name, corporations, partnerships, individual partners, voluntary unincorporated associations, individuals with guardians, and the executor or administrator of an estate. (The form does not cover class action lawsuits.) All the forms include a query table that allows you to enter data once and then click a button to populate the form. The automation will ensure that subjects and verbs agree, pronouns are correct, mathematical calculations are computed accurately, and dates are in the proper formatting. All the forms are also compatible with each other so that you can save data entered into one form and transfer it to another, saving valuable time in your forms completion process (click here for a video on how this works). Since the automation works right in Microsoft Word for Windows, you can also edit the forms at any point in the process.

The suite includes the following 21 forms:
